Palliative Care Co-ordination Centre – Patient Referrals
St Barnabas Hospice provides free, palliative care to adults aged 18 and over in Lincolnshire who have been diagnosed with a life-limiting or terminal illness.
The Palliative Care Co-ordination Centre (PCCC) is where patient referrals can be made. It is also available to Health Professionals, carers, patients and their families who need advice and support regarding end-of-life care.
The Palliative Care Co-ordination Centre is open 365 days a year. Operating from 9am to 6pm Monday to Friday and 9am to 5pm on Weekends and Bank Holidays.
Patients, family, friends and healthcare professionals can refer a patient to St Barnabas by calling the PCCC team on 0300 0200 694 once they have obtained consent. If a referral is declined, this will be communicated directly back to the person who made the referral.
Calls are initially taken by the administration team who will forward the referral to the relevant team. The clinician may need to contact you for further details.
