Many people want to be cared for in their own home. This is where they feel safe and comfortable and can spend precious time with their loved ones.

The Hospice at Home team support this choice and work 365 days a year to care for patients in their own homes across Lincolnshire

Working across the county, the team is made up of specialist doctors, registered nurses, physiotherapists, occupational therapists, wellbeing professionals, and health and rehabilitation support workers. The types of support offered can be physical, emotional, spiritual, and social.

What is Hospice at Home care?

The Hospice at Home team provides specialist care for people living with a life-limiting or terminal illness. When you are in the care of St Barnabas, you have access to a specialist group of clinicians who work together with you and your community team to ensure all aspects of care are met.

A personalised, patient-centred care plan is developed to reflect your wishes and choices. This includes symptom management and tailored support for each individual.

Patients and their families are supported to continue living with their diagnosis and to plan for their future care. This includes having important conversations about what matters most to you and how to let loved ones know your wishes.

How can the Hospice at Home team help?

Working collaboratively with our wellbeing services, we provide specialist care, including:

  • Holistic assessment of physical, psychological, social, and spiritual needs.
  • Working with you and your family to develop personalised care plans to achieve your goals and preferences.
  • Practical advice, education, support, and clinical interventions to make it possible for you to achieve your goals and preferences.
  • Highly skilled nursing and therapy care and advice to manage symptoms such as pain and fatigue.

The Hospice at Home team also work closely with your doctor, community nurses and other care services.

What is the criteria for Hospice at Home care?

To refer a patient for Hospice at Home care, they must meet the following criteria:

  • Registered with a Lincolnshire GP
  • Aged 18 years or over
  • Diagnosed with a life-limiting or terminal illness
  • Consent is given by the patient, wherever possible

The amount of care that you receive may increase or decrease depending on how your needs change. St Barnabas Hospice operates an open referrals service should a patient need us again in the future.

How can I access Hospice at Home support?

Self-referrals can be made via the phone line: 0300 020 0694. Family, friends, and informal carers can refer a patient to St Barnabas’ Hospice at Home service once they have obtained permission from the patient to do this.

Your doctor, community nurses and other healthcare professionals involved in your care can also refer to our services. For healthcare professionals to refer a patient to the Hospice at Home service, please complete the referral form and email to lhnt.lchsreferrals@nhs.net.

When referring, please include a detailed account of the patient, their symptoms, the complex symptom or situation, how they are presenting, the impact of the symptoms on their life and what is currently not being managed due to these symptoms.

Once a referral has been received, a member of our team will contact you and your family to confirm where best to meet you to complete your initial assessment. Your care will then be tailored to your specific needs.

The end of John’s life would have been so much harder for both of us if St Barnabas hadn’t been there.

It was thanks to the wonderful team at St Barnabas that my partner John was able to spend his final weeks at home, just as we wanted. From the first time John stayed at St Barnabas to the team’s support at home during the last three months of his life, it was obvious just how much everyone at the hospice truly cares. Over time, I lost myself and felt like I was just John’s carer. But for a few times a week, the St Barnabas team made me feel like more than that. The end of John’s life would have been so much harder for both of us if St Barnabas hadn’t been there. St Barnabas gave us the support we couldn’t get anywhere else. It’s the kind of care I’m sure everyone would hope for at the end of their life or a loved one’s.
