Care Referrals
St Barnabas Hospice provides free, palliative care to adults aged 18 and over in Lincolnshire who have been diagnosed with a life-limiting or terminal illness.
Wherever you want to be, we will be there for you.
The term ‘life-limiting or terminal illness’ includes conditions such as cancer, dementia, Parkinson’s, heart failure, and motor neurone disease. There are multiple options for patients who need specialist care.
We have two Inpatient Units, one in Grantham and one in Lincoln, which can help patients be looked after around the clock. We also offer Hospice at Home care around the county, helping patients be comfortable in their own homes.
Criteria for referral
- Registered with a Lincolnshire GP
- Aged 18 years or over
- Diagnosed with a life-limiting or terminal illness
- Consent is given by the patient, wherever possible
When referring, please include a detailed account of the patient, their symptoms, the complex symptom or situation, how they are presenting, the impact of the symptoms on their life and what is currently not being managed due to these symptoms.
