Care Referrals
St Barnabas Hospice provides free, palliative care to adults aged 18 and over in Lincolnshire who have been diagnosed with a life-limiting or terminal illness.
Criteria for referral
- Registered with a Lincolnshire GP
- Aged 18 years or over
- Diagnosed with a life-limiting or terminal illness
- Consent is given by the patient
When referring, please include a detailed account of the patient, their symptoms, the complex symptom or situation, how they are presenting, the impact of the symptoms on their life and what is currently not being managed due to these symptoms.
How to refer: Hospice at Home
Patients, family, friends and informal carers can refer a patient to St Barnabas Hospice at Home service by calling 0300 020 0694 once they have obtained consent from the patient to do this. If a referral is declined, this will be communicated directly back to the person who made the referral.
For healthcare professionals to refer a patient to the Hospice at Home service, please complete the updated referral form and email to lhnt.lchsreferrals@nhs.net.
How to refer: Inpatient Unit
Referral to our Inpatient Unit in Lincoln must be made via a healthcare professional such as your GP, specialist nurse, community nurse or hospital.
For healthcare professionals to refer a patient, please complete the IPU referral form and email to: ipureferrals@stbarnabashospice.co.uk. Please be advised that St Barnabas Lincolnshire Hospice have now closed all of their NHS.Net accounts and will only be using their St Barnabas Microsoft outlook accounts.
