Sharing Your Story

Thank you for agreeing to have your photograph taken, to appear in a video, or to share your story with us. Each year, St Barnabas Hospice covers dozens of stories told to us by our patients or their loved ones, our supporters and corporate sponsors. Photographs and videos also allow us to promote the work that St Barnabas does each and every day across Lincolnshire.

Before you complete the Sharing your Story Consent Form, we would like to make you aware of the following:

  • By signing this form, you agree that your story (in full or in part), photographs, and/or video may be used in printed materials, online content (including our website and social media), presentations, and other communications.
  • Once shared, your story may be used by external media such as newspapers, websites, radio, or TV, which is beyond our control.
  • St Barnabas Hospice cannot be held responsible for any objections raised by third parties after content has been shared with your consent.
  • You are under no obligation to give consent and should only do so if you feel completely comfortable.
  • We encourage you to inform your next of kin. They may contact us to withdraw consent at any time.
  • If consent is withdrawn, we will stop future use and do our best to limit further sharing; however, we cannot guarantee removal from materials already published or distributed.
