Sharing Your Story

To promote our many services on our website, social media and in print, we need captivating photos and videos. As St Barnabas is known for that personal touch, we love to show the people behind the charity; all you incredible staff members and volunteers.

Before you complete the Sharing your Story Consent Form, we would like to make you aware of the following:

  • By completing this form, you agree we can share photos and / or videos of you on electronic communications, presentations or other media like our website and social media.
  • You understand that once your photo / video is shared, it may be shared further by press outlets – this could include news websites or newspapers, radio, TV, or other media.
  • St Barnabas cannot be held responsible for any objections made by third parties (such as family members) once your photo / video has been shared, with your consent.
  • You should only agree to us using your photo/film/words if you feel completely happy with us doing so. You are under no pressure or obligation to complete the form.

I am happy for my story, photos and/or video to be shared on social media and our website. I understand this may be shared further by other news outlets, which is beyond the control of St Barnabas Hospice.

I am happy to have my story, photos and/or video to be sent as press release directly to news outlets.

I am happy to have my story, photos and/or video to be used as part of grant applications to generate further income for St Barnabas Hospice.
