Complete this form to dedicate a light in memory of someone you will never forget this Christmas time. Your light will shine brightly on the Tree of Life outside Lincoln Cathedral.

Our Tree of Life will be illuminated from the 19th November 2024 to early January 2025. It is free to see and no appointment is needed.

For dedications received on or before 20th December, we will send a personalised card, a memory tag for the Tree of Life, and a decoration for you to display in your own home.

Please note: When you dedicate a light, some or all of the following information will appear on our website for other Light up a Life supporters to view: your full name, the full name of your loved one and your written tribute.
