Complete this form to dedicate a light in memory of someone you will never forget this Christmas time. Your light will shine brightly on the Tree of Life outside Lincoln Cathedral.

Our Tree of Life will be illuminated from the 18th November 2025 to early January 2026. It is free to visit and no booking is needed.

For dedications received on or before 19th December, we will send a personalised card, a memory tag for the Tree of Life, and a decoration for you to display in your own home.

Please note: When you dedicate a light, some or all of the following information will appear on our website for other Light up a Life supporters to view: your full name, the full name of your loved one and your written tribute.
